Choice Request Form-All students that do not live in the West Valley School District are required to fill this out every year. If you are not sure what district you live in, please fill it out and the office will determine your resident district.
Family Income Survey: this is required every year for any student that attends DHHS regardless if they plan to eat school meals or not.
